Would it be too speculative to suggest that the "risk on risk" bit in that report arose from attempting to keep ships in service under ever decreasing support budgets thanks to the pressure the MoD funding finds itself in?   Puts Gordon Brown's remarks about giving the Service Chiefs what they wanted in context perhaps?!

Not really.
The fault that caused the flooding was improper training and pneumatic actuating pipes connected to wrong valves but not picked up.

The inservice story is a junior stoker, thought it would get brownie points by unblocking a main sea inlet by taking it off without telling anyone.
