For those with the time to read it, this is a very interesting report on last year's fire on Commodore Clipper. A very good illustration of how problems can pile up from a simple original cause - in this case the somebody hsd wired up a plug incorrectly.

Until recently, I always thought "a catalogue of errors" was down to incompetence of the like, but having been involved in a 'missing new laptop' saga,
 I now understand a little how these things happen.  Briefly:

X number of boxes delivered.
Mail room signs for boxes without counting as he's a trusted courier and he delivers 2 or 3 times a week.
I collect boxes from mail room and start to process order.
I find a discrepancy and ring end customer to clarify.
End customer "clarifies" that order has already been fulfilled.
All order items accounted for and order completed and filed.
A month later, 'end customer' rings up to enquire about his outstanding new laptop.
"You said the order had been fulfilled?!"
"Ah. I thought you was talking about something else. My laptop is still outstanding."
"??? OK, I'll investigate."
Two days of going through everything reveals that there was one box short on the delivery but was mistakenly signed for by the mail room.
We go through the CCTV and "sort of" conclude that there is a box missing but can't swear to it as, like most CCTV, they ain’t that good!
Question the courier:
“Not our problem chief, we got your signature!”
Question the supplier;
“Not our problem chief, it’s out side the 14 day limit.
Mail room forks out for a brand new laptop.
