If it's the standard one with a button on it, then this is the way to program it.

Centre the joystick
Switch on transmitter
Switch on Speed controller
Observe the lights flashing

You have 2 seconds to do the following

Push throttle stick fully up
Pull throttle stick fully backward
Return to centre position

Job done

If its the one with one button, it helps to do like Kenny says, but push the button within the two seconds.
It could be that it has been accidentally re-programmed (no idea how to get back to square one without the instructions), it could be broken.  Does it give "off" at stick centre?

The real end of the story could have happened in the mayhem trip-up of mid October.  About a weeks-worth of postings vanished.
