Only time I've seen this occur is with overcharging. NiMH deal with overcharging by dispersing the energy as heat. Does your charger cut off automatically or is it a constant supply (constant current/constant voltage)?
Generally I check NiMH packs frequently during charging and if they're noticeably warm I immediately stop charging.
If the pack had overheated during charging it's possible that a battery shorted internally and then the rest, as they say, is history.
These days I charge all my batteries (Lipo or NiMH) in an old metal oven tray. It serves to protect the surroundings if anything should go wrong. I've had a single Lipo and a single NiMH battery go. The Lipo I think was internally damaged, unclear whether that was my fault or manufacturing defect. The NiMH went while I was charging it with some new software I'd written and the software didn't cut off the charge current correctly. I'll take full blame for that one :)
