I would have said a case of a lack of common sense - but there is no such thing - apparently, so a lack of observational ability might be more correct.

The newspaper articles yesterday make no mention of sat nav just driver error so I guess that counts as a big mistake.  It's quite impressive though how they passed over all that arris rail with such small wheels. Gladly nobody was reported as hurt, just allot of folk in Gloucestershire probably very hacked off at the standstill of everything on the line at morning commute time.
