That's almost impressive! I can be cack-handed when I'm not thinking, but struggling to think how I could even run a die up a rod (even a small-one) so far off-centre without breaking the die, or the shaft in the first place. I'm sure a reputable UK supplier would not do that, but they might put out-sourced parts in a box without checking which says a bit also. I'm finding that very often -very much with prop-shafts for the last few years now.
...Whats the brass(?) extension covering the shaft outside of the shaft tube with the square-cut "bush" on the end though -that's a bit of an interesting arrangement I've not come across before? Not many suppliers of lifeboat kits these days so narrows things down.

............no, not in any way! But very probably very easily rectified by asking for a replacement, there is no denying that's not right.
